You would pick a Concealer shade based on your skin tone and skin undertones...not by the shade of your Foundation or Tinted Moisturizer.

If your skin tone is Medium, then you would look at the Medium Shades (M1-4) and then find the one that best suits your skin undertone.
But, you might need a Light-Medium (L1-4) shade or possibly a Medium-Tan shade (T1-4).

It's also really helpful to look at the models in the product photos to find someone close to your skin tone and undertone.

Medium Shade options:
*M1 is Light-Medium skin with a Neutral undertone.
*M2 is Light-Medium skin with a Peach undertone.
*M3 is Medium skin with a Neutral undertone.
*M4 is Medium skin with a Warm undertone.
